Initial assessment. Patient 6 weeks post R rotator cuff repair. Reports pain 3/10 at rest, 5/10 with reaching above shoulder. Still wearing sling intermittently. Self-referred — not covered by MSI.

R shoulder: flexion 120 deg (passive 140), abduction 100 deg, ER 30 deg (limited by surgical precautions). Scapular dyskinesis present. Grip strength R 22 kg (L 30 kg). No signs of infection at portal sites.

Recovery on track for 6 weeks post-op. Passive ROM near normal but active ROM limited by rotator cuff weakness and guarding. Scapular stability needs addressing. Patient motivated and compliant.

2x/week for 8 weeks. Phase: active-assisted to active ROM. Scapular stabilization exercises. Progressive rotator cuff strengthening starting week 8. HEP: pendulum exercises, wall walks, scapular squeezes. Private pay — $95/session.
