Patient reports R hip pain 4/10 at rest, 6/10 with stairs. Sleeping better — only wakes once at night. Using walker consistently.

R hip flexion 95 deg (was 80 deg at initial). Abduction 30 deg. Gait: antalgic pattern improving, heel strike present. TUG 18 sec. Scar healing well, no signs of infection.

Good progress 5 weeks post-op. ROM improving ahead of schedule. Gait pattern normalizing. Fall risk moderate — stairs remain a concern.

Continue 2x/week visits. Progress to resistance band exercises for hip abductors. Add stair training next visit with handrail. Update HEP with standing balance exercises.
