Subjective
Patient pleasant and cooperative. Staff report she has been more mobile this week — using walker to dining room independently. Mild R knee pain with prolonged standing.
Objective
Ambulation: 40m with rollator, minimal rest breaks. Sit-to-stand from wheelchair: independent with armrest push. R knee: crepitus present, no effusion. Grip strength adequate for walker use. Skin intact — no pressure concerns.
Assessment
Functional mobility maintained well for 85yo LTC resident. Walking tolerance stable. R knee OA symptomatic but not limiting function. Fall risk remains moderate — cognitive status stable.
Plan
Continue 1x/week maintenance visits. Focus on walking endurance and sit-to-stand strength. Monitor R knee — if worsening, recommend X-ray. Coordinate with nursing re: fall prevention protocol. Update care plan with facility.
