Objective
Berg Balance 46/56 (was 42). TUG 12 sec. Tandem stance 12 sec. Single leg stance R 10 sec, L 7 sec. Gait speed 1.0 m/s. Outdoor walk test: 200m on sidewalk with cane, no loss of balance.
Assessment
Continued balance improvement. All scores now in moderate-risk range, trending toward low-risk. Community ambulation initiated successfully. Grab bar installation has improved bathroom safety. Patient confidence growing.
Plan
Reduce to 1x/week. Community walking program — increase distance gradually. Add reactive balance training. Discuss group exercise class at community centre. Target discharge in 3-4 weeks if balance scores maintain.
