Subjective
Patient reports pain down to 3/10. Able to do heel slides to 100 deg at home. Riding stationary bike 10 min/day. Feeling more confident on stairs with railing.
Objective
R knee flexion 98 deg (was 72 deg at initial), extension -3 deg (was -8 deg). Effusion resolved. Quad lag 5 deg (was 15 deg). Gait: independent without device on flat, using railing on stairs. STS: independent from standard chair height.
Assessment
Excellent ROM recovery — ahead of typical post-TKR trajectory. Quad strength improving but still deficit. Functional mobility much improved. On track for discharge in 3-4 weeks.
Plan
Reduce to 1x/week. Progress to resistance training — leg press concept, step-ups, terminal knee extension with band. Add balance challenges. Outdoor walking program. Target discharge at week 8 post-op if ROM >110 deg and stairs independent.

Subjective
Initial assessment. Patient 2 weeks post R TKR. Reports pain 5/10 constant, 7/10 with movement. Difficulty with sit-to-stand and stairs. Lives alone in condo — elevator access.
Objective
R knee flexion 72 deg, extension -8 deg. Moderate effusion. Wound clean, staples removed. Gait: using rollator, short step length, guarded pattern. SLR possible but weak. Quad lag 15 deg.
Assessment
Typical post-TKR presentation at 2 weeks. ROM below expected — flexion target 90 deg by week 4. Quad activation poor. Independent ambulation with rollator on flat. Stairs require railing + supervision.
Plan
2x/week for 6 weeks. Priority: ROM (flexion + extension), quad strengthening, gait normalization. Begin CPM-equivalent exercises at home. HEP: ankle pumps, quad sets, heel slides, SLR. Review in 2 weeks for progress check.
