Subjective
6-week progress review. Patient reports L hand function much improved — can button shirt, hold fork. Walking with quad cane outdoors. Wife reports he is more independent with morning routine.
Objective
L UE: shoulder flexion 130 deg, grip strength 14 kg (was 8 kg). L LE: hip flexion 110 deg, ankle dorsiflexion 10 deg (was 0). Berg Balance 44/56 (was 38). 6MWT: 220m (was 150m). Transfers: independent all surfaces.
Assessment
Significant functional gains at 6-week mark. L UE grip strength nearly doubled. Balance scores improved to moderate risk range. Walking endurance up 47%. Patient exceeding expected recovery trajectory for age and stroke severity.
Plan
Continue 2x/week for 4 more weeks then reassess. Focus shifting to community reintegration — outdoor walking, public transit training. Coordinate with OT re: kitchen and bathroom modifications. Begin discharge planning for week 12.

Patient reports improved L arm control this week. Able to hold cup with L hand briefly. Fatigue remains a barrier — best in mornings. Wife notes improved mood.
Objective
L UE: shoulder flexion 110 deg, grip strength 8 kg (R=32 kg). L LE: hip flexion 100 deg, knee extension -5 deg. Berg Balance 38/56. Transfers: mod-I with standby assist.
Assessment
Steady neurological recovery 4 weeks post-stroke. L UE function improving faster than anticipated. Balance still impaired — fall risk high. Fatigue management key.
Plan
Continue 2x/week. Focus on functional L UE tasks — reaching, grasping. Standing balance exercises with perturbation training. Coordinate with OT for ADL retraining. Morning appointments preferred.
